MEMBER BENEFITS

Dr. Rosen will likely continue to accept your insurance. In addition, Program Fees will pay for services not covered by your insurance. This section highlights some of these benefits.

SHORTER WAIT TIMES

By capping the number of members and expanding office hours wait times will plummet. Program membership does not enable you to metaphorically “cut to the front of the line” but is expected to very much shorten the line (or required wait time).

24/7 Connection TO DR ROSEN 

You can text Dr. Rosen for urgent issues when they arise. For emergencies, you can call 911 and then text Dr. Rosen.

Same Day Appointments 

Illness is not scheduled. When you are not feeling well, the last thing you want to do is have to wait for days to be seen by your physician. Dr. Rosen will make every effort to see you on the same day or the next day. The smaller patient panel, compared to traditional practices, enables this result.

2-HOUR PHYSICAL EXAM

Each year, you will have access to a 2-part physical exam, with each session lasting about an hour to address both the normal aspects of your health as well as other aspects of wellness that the current system does not enable. Wellness initiatives may partner with you focusing on behavioral modification or health coaching. Your membership fee is for the portion focusing on wellness not covered by insurance. For Medicare beneficiaries it includes a routine comprehensive physical exam and select issues that will be molded to your interests. You will find several examples below. For commercially insured patients, the wellness exam focuses on extended heart health evaluation as well as other issues that will be molded to your interests. Here are several examples:
a. Heart health (beyond medicines)
b. Brain health
c. Exercise / balance
d. Hearing and vision
e. Nutrition
f. Weight management
g. Sleep health
h. New cancer screening
i. Medication reviews

The latter portion of your physical will include traditional medical issues and may be covered by your insurance.

Additionally, membership will include follow-up in-person or virtual routine exams intended to support ongoing health & wellness goals identified in the routine comprehensive exam that are not covered by insurance.

COMPLETING FORMS NOT COVERED BY MEDICAL INSURANCE

These might include forms associated with travel or with travel insurance, sports participation forms, and others.

PRESCRIPTION RENEWALS

You will not be required to make an office appointment to get a prescription renewal or pay a fee for this. Medicines provided will be those in the scope of Physician’s comfort and exclude those for whom you should get from a specialist.

FREQUENTLY ASKED QUESTIONS

In order to devote more time to each patient’s care and individual needs, the number of patients is capped. Dr. Rosen also offers certain non-covered amenities and benefits designed to personalize and enhance the health care experience. In-office appointments are expected to start promptly, with little or no waiting time. This practice model also enables scheduling longer patient appointments (approximately 30 minutes depending on the needs of the patient and situation) and 120 minutes for Annual Physical Exams. This is much different than the average 7 to 15 minute visit provided for traditional practices for routine care and 20 to 30 minutes for physicals. If your issue requires extra time for evaluation or discussion, Dr. Rosen will accommodate you to the best of his ability. Also, after-hours you will be able to contact Dr. Rosen via text for urgent issues making it easier than ever to communicate.

Concierge medicine is a different model, where the relationship is directly between the patient and the doctor. No insurance company, no nurse gatekeeper, just good old-fashioned medicine. The goal is no waiting on the phone, no redirecting, no delay in being seen, no layers, no hassle. Our approach is “yes, how can we help?” Your fee provides for services not included in your insurance (e.g. services Medicare does not cover or your Commercial insurance excludes).

All of the local hospitals now use hospitalists which means Dr. Rosen does not admit to any hospital. This has been the norm for over two decades.  Dr. Rosen is on staff at Alta Bates and has access to their medical records and those of John Muir Hospital. Should you need to be hospitalized, Dr. Rosen will be able to communicate with the hospitalists who care for you. If you are hospitalized, he will do his best to be available to communicate with you, the hospitalist caring for you, and to serve as your advocate.  

Dr. Rosen intends to remain an in-network provider for many major PPO insurance plans and will bill your insurance directly for professional services that are covered by those plans. Professional services are not covered by your annual fee. If the terms of your insurance plan require a co-pay, the practice is obligated to request payment at the time of service. Even if Dr. Rosen is not an in-network provider for your insurance plan, he will attempt to refer you to in-network physicians for any necessary consultations and to in-network facilities for diagnostic tests and hospitalizations as the need arises. Those services will likely be covered by your insurance plan.

Yes. The office will file your claims with Medicare as well as with your supplemental PPO insurer on your behalf, as required by law. Office visit fees that are not reimbursed by insurance will be the responsibility of the patient.

No. Though not covered, it will be a deductible medical expense for those who itemize medical expenses on their tax returns. Check with your tax advisor if you have specific questions.

The annual fee is $2,500 for 2024 which is equivalent to a little more than $200 month.

Your annual fee may be paid in full by check to: Rosen Personalized Medicine. Also, if preferred, it may be paid annually, semiannually, or quarterly by credit card. If you opt to pay by credit card, the first payment will be charged to the credit card you indicate on your Membership Agreement Form. The remaining balance of your annual membership/enrollment fee (if any) will be charged automatically to your credit card in installments after your start date, accordingly. Until we hear otherwise, payments will be processed at the same frequency.

With far fewer patients, Dr. Rosen can spend more time focusing on prevention and wellness than a physician in a traditional practice possibly can. You won’t find a crowded waiting room brimming with contagion. Visits can be longer. Not only will there be time for him to ask questions, but time also to express your answers.

Dr. Rosen performs comprehensive physical exams to detect issues in their early stages. If you happen to be away on a business trip or on vacation, Dr. Rosen is still just a phone call away.

Our team will go the extra distance when a referral to a specialist is warranted.

You will text Dr. Rosen’s cell phone. If I don’t have a cell phone other arrangements will be made.

No. For urgent issues only, Dr. Rosen is available by text. Your issue may be taken care of at that time. He may also suggest an appointment usually that same day or the next day to address what is going on.

Knowing you (and your family) well and having access to your electronic medical records, Dr. Rosen can conduct a video visit so long as you have a smart cell phone or a computer and internet. If that’s not available, we can conduct a phone appointment.

No. This is not a form of insurance. You will need to maintain insurance to pay for office visits, for specialist care, for hospitalizations, outpatient procedures, labs, radiology, etcetera.

Routine Exam Services costs are designed to qualify as eligible medical expenses such that Patient may pay Services Fees with health saving account (“HSA”) funds or with flexible spending account (“FSA”) funds or health reimbursement account (“HRA”) funds, or similar funds, but this is not assured or promised. Patient must confirm eligibility with Patient’s tax expert or FSA/HRA plan coordinator as Practice cannot guarantee medical expense eligibility due to variable factors applicable to each Patient.

No. All medical services and procedures performed in the office will be billed by the performing physician and/or provider. Costs you incur for labs, radiology, specialists, and hospitalizations outside the office will not be paid by your membership fee.

Yes. Your annual fee allows you to be a member of the practice and to text Dr. Rosen for urgent health matters. You are encouraged to use the benefits offered and to proactively maintain & improve your health.

Your membership agreement can be terminated upon 30 days written notice to Rosen Personalized Medicine. If the extended comprehensive annual exam is not performed, you may be eligible to receive a prorated refund. If you have received the exam, a refund is not available and any unpaid balance of the annual fee remains your responsibility.

Yes. Dr. Rosen enjoys getting to know and care for families. This start up provides a special opportunity to allow family members in, as his practice had previously been closed to accepting new patients. Children ages 18 to 25 are included for no additional charge if both parents are members.

Yes. Provide a letter to Dr. Rosen describing the circumstances, your federal income tax filing for the past 2 years, and your W-2, 1099s, and K-1’s, whatever is applicable. The determination is at the sole discretion of Dr. Rosen.
